| More than 60 providers are scheduled to attend Imagine!'s 2013 Provider Fair |
The Provider Fair will include raffle prizes, giveaways, light refreshments, and opportunities to meet with Imagine! coordinators, navigators and case managers. There will be opportunities to learn about the Children’s Medicaid Buy-In Program and the new Colorado Community Health Alliance, as well as family and self-advocacy groups.
More than 60 providers will be at the Provider Fair, including individuals who contract with Imagine!, Program Approved Service Agencies (PASAs), and community organizations offering direct services and supports (such as various therapies, day habilitation, employment, respite care, summer camps, recreation and more). Providers include those who offer services through the following sources:
- Early Intervention (EI)
- Autism Spectrum Disorder (ASD)
- Families Most In Need (FMIN)
- Children’s Extensive Services (CES)
- Family Support Grants (FSSP)
- Boulder County Initiative (BCI)
- Supported Living Services (SLS)
- Private Payment
When: Thursday, April 18, 2013
Where: Imagine! John Taylor Conference Center, 1665 Coal Creek Drive, Lafayette, CO 80026 MAP and directions
Time: 4:00pm to 7:00pm
